For your FREE EVALUATION simply snap a few photos of you dents, enter the
requested information in the response form below and click the SUBMIT button. A
new page will appear. Click the BROWSE button and locate the photo you want to
send-then click the UPLOAD button, the upload button will flash indicating your
photo is being attached, you may upload five photos. When you have uploaded
your photos click the CONTINUE button. A new page will appear with a cryptic set
of numbers and letters, enter them in the box and click the SUBMIT button. A
pop-up box will appear indicating the email has been sent and a confirmation
email will be sent to your email box confirming your request. I will review them
